About the Role
Full-time home health Licensed Practical Nurse (LPN) position in Bayonne, NJ, serving Hudson County.
Responsibilities
- Provide 1:1 patient care following individualized care plans while maintaining dignity and independence
- Administer medications, feedings, oxygen, and ostomy care, and assess results
- Accurately document nursing actions and communicate with family, case managers, and physicians
- Update on patient progress and coordinate supplies and medications per physician instructions
Requirements
- Current unrestricted LPN license in the practice state
- Valid driver’s license, acceptable driving record, and proof of insurance
- CPR certification
Qualifications
- Preferred: 1+ year nursing experience
- Home health experience is a plus
