Licensed Practical Nurse - Home Care

About the role

Assist in delivering patient-centered, community-based nursing care under the supervision of Registered Nurses. Provide personalized, clinical care to patients in their homes, monitoring health status, and adjusting care plans accordingly. Communicate effectively within interdisciplinary teams, coordinate supplies, and educate patients and families on health management and end-of-life care. Utilize technology for accurate record-keeping and care coordination.

Requirements

• Licensed and registered as an LPN in New York State
• High School Diploma or LPN Program completion
• Minimum one-year LPN experience preferred
• Valid driver’s license may be required
• Bilingual skills and home care experience are advantageous
• For certain roles, experience in mental health or hospice care may be preferred

Pay

$38.23 - $40.00/hr or $50.68 - $54.07 per visit (per diem).
