Role Overview

Responsibilities

• Review patient histories
• Administer immunizations
• Perform blood pressure assessments
• Support telehealth activities such as data collection and prescription research
• Assist in patient assessment
• Communicate findings to RNs or providers
• Ensure care delivery aligns with organizational policies and patient care plans

Qualifications & Skills

• Graduate of practical nursing program or passed LPN Boards
• One year of recent LPN experience preferred
• Strong communication, adaptability, and teamwork skills
• Ability to work effectively in a dynamic environment and innovate care approaches
• Current LPN license and BLS certification (or equivalent)
• Additional state-specific licenses or certifications as required
