Position Summary
Capital Area Health Network (CAHN) is seeking a Licensed Practical Nurse (LPN) to support the Cessation Education and Social Environment (CEASE) Initiative. This one-year pilot program aims to address the significant need for smoking cessation support in the Richmond City Community and surrounding counties.
Essential Functions And Responsibilities
- Triage patients interested in smoking cessation information, resources, and/or enrollment in CEASE program.
- Evaluate referred patients for readiness to quit smoking and identify potential/current barriers to cessation.
- Coordinate care and resources with the interdisciplinary team to ensure patient safety and achieve program goals.
- Establish daily, weekly, and monthly goals with enrolled CEASE Program patients and conduct health/educational assessments.
- Facilitate in-person individual/group sessions as scheduled by the team.
- Travel to sites as needed to provide direct patient care, triage, and other duties.
- Participate in case conferences with the interdisciplinary team and provide supporting facts.
- Assist physicians with individual health education and provide relevant literature.
- Assist in developing policies, procedures, and clinical protocols.
- Conduct monthly quality assurance checks and maintain appropriate par levels on medications and medical equipment.
- Prepare monthly, quarterly, and annual reports in a timely manner.
- Aid in the development of orientation materials for new employees and serve as a nurse preceptor.
- Assist the RN and DON with administrative tasks as assigned.
- Empower clients to make healthy decisions and improve their quality of life.
- Maintain accurate patient records in accordance with HIPAA regulations.
- Process routine, urgent, and STAT referrals.
- Attend team meetings and provide clinical updates as necessary, and prepare training materials.
- Perform other duties as assigned.
Position Requirements
- Valid Virginia driver's license, satisfactory driving record, and reliable transportation.
- Licensed to practice in Virginia with current CPR (Healthcare Provider) certification.
- At least three years of nursing experience, preferably in a community health/public health setting.
- Leadership skills and demonstrated knowledge, skills, and critical thinking abilities.
- Computer literacy and experience with computerized charting.
- Flexible scheduling, including working 5:00pm-9:00pm on weekdays and 8:00am-12:00pm on Saturdays.
