Responsibilities
- Provides direct and indirect nursing care individualized to the patient’s needs
- Collects and records relevant patient health data accurately
- Evaluates health data to identify patient problems
- Implements and documents specified interventions
- Identifies deviations from expected patient outcomes
- Adheres to approved, research-based practice standards
- Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures
- Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures
- Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights
- Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity
- Assures that patients have informed consent signed for indicated procedures
- Maintains current licensure, certification, and competencies
- Provides proof of current BLS validation to supervisor
- Completes required unit credentialing according to departmental policy
- Provides to supervisor evidence of completion all required annual training and competency validation
- Practices within the New York State-defined LPN scope of practice 100% of the time
- Acquires and maintains current knowledge and skills in his/her area of practice
- May be required to make home visits
- Functions as part of the interdisciplinary health care team
Requirements
- Graduate of a Practical Nursing program
- Current NYS LPN License and registration as a Practical Nurse or a valid interim permit
- BLS certification must be obtained within 6 months of hire

Physical Requirements
- Heavy Work - Exerting up to 100 lbs. of force occasionally, and/or up to 50 lbs of force frequently, and/or up to 20 lbs of force constantly to move objects

Pay
- Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards
- Pay Range: $24.74/Hr. – $37.10/Hr.
- LPN sign-on bonus: $10,000 for full time
