Licensed Practical Nurse

Responsibilities

• Provide compassionate, whole-person care by building relationships and coordinating with healthcare providers
• Conduct triage and identify health concerns early
• Administer medications and support clients in managing chronic conditions
• Maintain accurate documentation and contribute to a recovery-oriented treatment environment
• Serve as a clinical resource and communicate effectively across teams
• Ensure client engagement, follow up on medical concerns, and uphold compliance with standards

Qualifications

• Active Oregon LPN license
• Minimum two years of nursing experience
• Valid driver’s license and ability to travel within the community

Preferred experience includes behavioral health, psychiatric nursing, and care coordination.
