About the role
Licensed Practical Nurse (LPN) in a skilled nursing and rehab community.
Responsibilities
- Providing direct bedside care to residents with compassion and professionalism.
- Administering medications and treatments as prescribed.
- Monitoring vital signs and resident conditions.
- Accurately documenting care in electronic records.
- Collaborating with RNs, physicians, and therapists to develop and support individualized care plans.
- Educating residents and families about health and care procedures.
- Supporting activities of daily living (ADLs) and responding promptly to any changes in resident condition.
Qualifications and Skills
- Current license as a Licensed Practical Nurse.
- Strong clinical skills and attention to detail.
- Excellent communication and teamwork abilities.
- Ability to provide compassionate, resident-centered care in a professional environment.
