Licensed Practical Nurse (LPN) in a skilled nursing and rehab community.

Responsibilities

• Providing direct bedside care to residents with compassion and professionalism.
• Administering medications and treatments as prescribed.
• Monitoring vital signs and resident conditions.
• Accurately documenting care in electronic records.
• Collaborating with RNs, physicians, and therapists to develop and support individualized care plans.
• Educating residents and families about health and care procedures.
• Supporting activities of daily living (ADLs) and responding promptly to any changes in resident condition.

Qualifications and Skills

• Current license as a Licensed Practical Nurse.
• Strong clinical skills and attention to detail.
• Excellent communication and teamwork abilities.
• Ability to provide compassionate, resident-centered care in a professional environment.
