Role Overview

Seeking a compassionate Physical Therapist to provide personalized rehabilitative care in a skilled nursing environment. You will work directly with residents to improve mobility, strength, and balance, guiding tailored exercises, tracking progress, and educating patients and families on therapy goals and home exercises. Collaboration with an interdisciplinary team and adherence to safety standards are essential.

Responsibilities

• Deliver individualized therapy sessions to residents
• Develop and implement care plans based on patient needs
• Document progress and update treatment strategies
• Educate patients and families about therapy and home exercises
• Maintain a safe and clean treatment environment
• Collaborate with healthcare team members
• Stay current with ongoing training and professional development

Qualifications & Skills

• Active Colorado Physical Therapist license (required)
• Degree from an accredited Physical Therapy program
• 1+ year of experience preferred; new graduates welcome
• Strong communication, attention to detail, and documentation skills
• Ability to work independently and as part of a team
• Physical ability to support patient mobility and equipment handling

Compensation & Benefits

• Hourly pay range: $45–$48 DOE, paid biweekly
