Licensed Occupational Therapist (OT) - Full-Time

About the role

Continuum Therapy Partners is seeking a Licensed Occupational Therapist (OT) to join our rehabilitation team serving Morningside Health Center and Sheboygan Progressive Center in Sheboygan, Wisconsin. This full-time opportunity allows you to provide occupational therapy services in skilled nursing facilities while helping adult and geriatric patients regain independence and improve their quality of life.

Responsibilities

• Evaluate adult and geriatric patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services focused on improving activities of daily living (ADLs), upper extremity function, mobility, cognition, and functional independence.
• Complete patient evaluations, progress notes, reassessments, and discharge documentation.
• Collaborate with Physical Therapists, Physical Therapist Assistants, Speech-Language Pathologists, nursing staff, physicians, and caregivers.
• Educate patients and families on adaptive equipment, home safety, and therapeutic exercises.
• Maintain accurate documentation in accordance with Medicare, state, federal, and company guidelines.
• Participate as an active member of the interdisciplinary rehabilitation team.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Wisconsin Occupational Therapist (OT) license required.
• Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or outpatient settings is preferred but not required.
• New graduate Occupational Therapists are encouraged to apply.
• Strong communication, clinical reasoning, and teamwork skills.
