About the role

Reema Health is a social-care and behavioral-health provider that delivers a personalized, community-based approach to care. We are seeking an experienced and self-motivated Licensed Mental Health Therapist to serve the behavioral healthcare needs of our members through a value-based, tech-enabled model. This role is remote, primarily via telehealth, with occasional in-home or community visits as needed. You will be part of a multidisciplinary Integrated Care Team—including Psychiatric Nurse Practitioners and non-clinical Community Guides—providing holistic, member-centered care for individuals with complex health and social needs.

Responsibilities

  • Assess and diagnose behavioral health conditions, both chronic and acute.
  • Formulate treatment plans in consultation with the collaborative health team.
  • Ensure a behavioral health perspective is incorporated into comprehensive care planning for complex patients.
  • Complete initial intakes, assess patient risks and needs, deliver therapeutic interventions, and refer to community resources.
  • Collaborate with primary care providers, psychiatrists, social workers, and other professionals to discuss treatment plans and progress.
  • Provide psychoeducation to members, emphasizing mental health promotion and disease prevention.
  • Support referral for members needing additional specialty behavioral health care (e.g., inpatient facilities) and advise on treatment plans until engagement.
  • Establish and foster trusting relationships with patients, aligning care with their goals and values.
  • Participate in multi-disciplinary case conferences to ensure holistic and appropriate care.
  • Provide coaching and education to clinical and non-clinical team members about supporting patients with behavioral health issues.
  • Support post-discharge planning to prevent readmissions.
  • Document care and coordination in the EMR in a timely and appropriate manner.
  • Contribute to the design and implementation of clinical pathways and protocols for therapeutic treatment of behavioral health conditions.

Requirements

  • LCSW, LMFT, or LPC with an active license in New Jersey.
  • Ability to cross-license in Ohio, Florida, and Virginia.
  • 3+ years of outpatient experience working with adults enrolled in Medicaid, Medicare, or dual-eligible programs.
  • 3+ years of experience assessing, planning, and managing behavioral patient care, including direct therapy and care/case management.
  • 3+ years of experience working with individuals who have significant mental and chemical health needs, with a belief in recovery.
  • 1+ years of experience practicing telemedicine.
  • Experience with underserved populations facing socioeconomic barriers to healthcare.
  • Excellent working knowledge of trauma-informed and evidence-based psychotherapy for mental and chemical health conditions.
  • Comfort working in an integrated behavioral health model alongside Community Guide team members.
  • Proficiency with electronic medical records and technology-based tools.
  • Ability to make quick and accurate clinical assessments of mental and behavioral health conditions.
  • Strong judgment in evaluating situations and handling difficult or confrontational scenarios calmly and equitably.
  • Ability to work effectively in a team, interfacing with patients, providers, and administrative staff.
  • Fluency in Google Workspace applications (Gmail, Docs, Sheets, Slides, Drive, and Meet).
  • Comfort working in web-based platforms and apps across mobile and desktop, with adaptability to new digital tools.
  • Demonstrated ability to independently troubleshoot technical issues and escalate as needed.
  • 18 years of age or older.
  • Compliance with federal, state, and local regulations regarding patient confidentiality and HIPAA.

Pay

$80,000 – $90,000 USD per year (New Jersey).

Schedule

Full-time (40 hours per week), Monday–Friday, with occasional evenings and/or weekends.
