Position Overview
As a Licensed Mental Health Professional, you will work collaboratively as part of a multidisciplinary Primary Care team to address the mental and behavioral health needs of patients. Your role will include:
- Provide same-day behavioral health consultations during primary care visits.
- Conduct brief functional assessments of behavioral health concerns.
- Deliver brief, evidence-based interventions (typically 1-6 visits).
- Address common primary care behavioral health needs including: Depression, Anxiety, Stress, Substance use, Chronic disease self-management, Sleep concerns, Health behavior change (medication adherence, diet, exercise).
- Support patients with chronic medical conditions impacted by behavioral health factors.
- Provide crisis assessment and connection to resources when needed.
- Work closely with Primary Care Providers (PCPs), nurses, care managers, and other team members.
- Participate in warm hand-offs and curbside consultations.
- Provide real-time recommendations to medical providers.
- Aid in behavioral health triage and care planning.
- Document consultations and interventions in the electronic health record (EHR).
- Maintain documentation that supports integrated, brief care delivery.
- Track patient outcomes and participate in population health management.
- Participate in team huddles and case consultation.
- Provide education to medical staff on behavioral health topics.
- Support development and maintenance of PCBH model fidelity.
Position Requirements
- Master’s degree or higher in social work, counseling, marriage and family therapy and licensed to practice independently in the state of Pennsylvania (LCSW, LPC or LMFT).
- Knowledge of Trauma Informed Care, Motivational Interviewing, Recovery and Illness Management practices, etc.
- Interest and/or experience providing short-term, goal-directed therapy.
- Ability to document a shared electronic health record and communicate effectively in a fast-paced, primary care clinic.
- Bi-lingual (Spanish), preferred.
