Licensed Medical Provider: Nurse Practitioner (NP) or Physician's Associate (PA)
About the role
The Licensed Medical Provider at Central City Concern may provide primary care services appropriate to the needs of the population we serve in a variety of different settings. The role will understand and be comfortable with treatment of physical health, mental health, and substance use disorder diagnoses in the context of care to a homeless and underserved population.
Responsibilities
- Work as a member of the primary care team to deliver comprehensive, patient-centered services.
- Take appropriate health histories, perform physical exams, review vitals, labs, other studies, prescribe medications, perform procedures, review outside medical records and other forms of treatment as necessary and appropriate to the standard of care being provided in the setting for both acute and chronic health concerns.
- Triage, diagnose, manage minor and/or acute illnesses and initiate treatment.
- Along with the care team, support patients in achieving health resiliency and engagement by utilizing techniques such as shared decision making, motivational interviewing, continuity relationship building, health education, and preventive care appropriate to patient characteristics.
- Timely completion of appropriate medical records.
- Attend all mandatory CCC training in a timely manner.
- Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
- Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.
- Provide after-hours phone coverage on scheduled rotation with colleagues.
- Collaborate with the care team to ensure optimal performance on team-based performance metrics.
Requirements
- Demonstrated skills in primary care medicine experience preferred.
- Must meet CCC privileging requirements as required by FTCA.
- Must have full prescriptive powers in Oregon with a DEA number.
- Must have current BLS certification.
- Wound care experience highly preferred.
- Spanish language fluency preferred.
- Valid driver's license preferred.
Qualifications
- NURSE PRACTITIONER: Graduate of an accredited Nurse Practitioner Program with the appropriate scope of training and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure. Appropriate board certification.
- PHYSICIAN ASSOCIATE: Graduate of an accredited Physician Associate Program and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure. Board certification with NCCPA.

- Able to interact with co-workers, patients and staff to instill confidence and facilitate communication.
- Experience in primary care or mobile medical practice.
- Ability to consider the impacts and outcomes for underserved communities during decision-making processes with attention to trauma informed principles, accounting for issues relating to social equity.
- Ability to display a non-judgmental attitude.
- Sufficient manual dexterity and physical ability to perform assigned tasks with reasonable accommodation as needed.
- Ability to manage time and meet deadlines.
- Knowledge of de-escalation methods or willingness to be trained in these methods.
- Comfort working in an independent clinical practice with on and off-site support from medical and administrative supervisors.
