Crisis Intervention
- Maintain assigned caseload
- Provide direct care to patients/residents including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio-psychosocial assessments
- Make referrals when appropriate
- Responsible for each assigned patients/residents' medical records
- All records are to be professional, complete and thorough with entries made in a timely manner
- Connect patients/residents with aftercare services prior to discharge and aid the resident and his/her family access services when appropriate
- Communicate/involving family in treatment at patients/residents' request
- Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Supervisor/Director
Duties and Responsibilities
- Provide clinical services to patients including required assessments, individual/family/couples counseling, group clinical services, discharge planning, treatment plans, progress notes, and completion of bio-psychosocial assessments
- Responsible for each assigned patients' medical records
- All records are to be professional, complete and thorough with entries made in a timely manner
- Complete all required external clinical communications including court/DCF/DMH monthly compliance reports, evaluation summaries, other requested external documentation
- Provide clinical services through in-person and telehealth modalities
- Connect patients with aftercare services prior to discharge and aid the patient and his/her family access outpatient services when appropriate
- Attend all identified meetings, MDT meetings and case conferences as directed by the Clinical Director
- Attend mandatory trainings and seek opportunities for clinical growth through certification
- Maintain professional license in good standing, as applicable
- Contact the insurance company for authorization, as needed
