About the role

CHD is seeking strong clinical professionals for the new ACCS program. Clinicians will lead person-centered treatment planning within multidisciplinary teams supporting Persons (age 18+) in a community based behavioral health program.

Responsibilities

• Lead person-centered treatment planning within multidisciplinary teams
• Support Persons in a community based behavioral health program
• Facilitate the planning and documentation of comprehensive assessments, treatment plans, and clinical services
• Model best practices and train the team in recovery principles
• Work collaboratively with integrated teams including licensed clinicians, RNs, housing specialists, direct care staff, and peer/family specialists
• Provide direct clinical interventions with Persons served

Requirements

• Master's Degree in a related field
• Licensed Practitioner of the Healing Arts (LICSW, LCSW, LMHC, Licensed Rehab Counselor, Physician, Registered Nurse, Registered OT, Psychologist)
• Three to five years of experience in a mental health treatment program
• Demonstrated experience working with Persons with psychiatric and/or substance use histories is preferred
