About the role
Unika Solutions Team is launching a Behavioral Health Integration (BHI) program embedded within primary care. We're looking for a licensed behavioral health clinician to serve as the clinical backbone of this initiative—working directly with patients, collaborating with primary care providers, and helping us build a scalable model from the ground up.
Responsibilities
- Deliver monthly behavioral health services to a panel of patients enrolled in Medicare primary care programs (APCM/CCM), starting with a small pilot cohort and growing over time
- Conduct initial clinical assessments using PHQ-9, GAD-7, and other validated screening tools; establish baseline scores and track patient progress
- Draft individualized behavioral care plans (goals, interventions, cadence, escalation triggers) per CMS BHI requirements (99484/G0570)
- Cook with primary care providers on patient status, flag escalations, and support monthly attestation workflow
- Document clinical contact in the care platform; maintain compliant time tracking for billing purposes
- Administer monthly behavioral health check-ins: structured follow-up, medication adherence support, goal review, care plan updates
- Participate in building the care plan template, escalation protocol, and clinical SOPs—helping define what this program looks like at scale
- Work within a two-layer staffing model: handle all clinical activities; Care Associates handle administrative and scheduling support
Requirements
- Active California license: LCSW, LPCC, or LMFT— in good standing, no restrictions
- 2+ years of clinical experience in behavioral health, preferably in a medical or integrated care setting
- Comfortable working in a primary care context—understand the Medicare/chronic disease population and can calibrate your approach accordingly
- Familiar with validated screening tools: PHQ-9, GAD-7; experience interpreting and acting on scores
- Organized and documentation-fluent—keep clean clinical notes and are comfortable with structured care plan formats
- Can work independently with providers you haven’t met before; low-ego, collaborative style
- Comfortable working remotely using standard technology (laptop, video conferencing, EHR, messaging platforms)
- Able to travel occasionally to Unika HQ in Sunnyvale for onboarding, training, and team meetings
Nice to have
- Medicare enrolled as a provider, or willing to enroll in the future (not required for BHI, which bills incident-to under the PCP’s enrollment; valued for future Unika programs)
- Experience with CMS BHI billing codes (99484, G0570) or familiarity with CCM/APCM programs
- Background in Collaborative Care Model (CoCM) or AIMS Center
- Stepped Care Experience in telehealth or distributed care settings
- Prior experience working with Care Associates, RDs, or other non-licensed care team members
