Licensed Clinical Social Worker (LCSW) - Full-Time

Role & Responsibilities

• Provide discharge planning and psychosocial interventions through assessment, advocacy, referral, and interdisciplinary collaboration.
• Prepare social histories and assess patients' social, psychological, emotional, and financial needs.
• Manage policies for setting and evaluating residents' long- and short-term goals.
• Offer social casework to individuals and families, supporting health and social functioning.
• Afford assistance to healthcare teams in understanding significant social and emotional factors affecting patients.
• Document services in compliance with applicable regulations and payer guidelines.

Qualifications & Skills

• Must possess an active LCSW license (most travel/contract roles require this).
• Minimum of 2 years of relevant work experience.
• Strong decision-making, communication, and interpersonal skills.
• Preferably hold a Master’s degree from a CSWE-accredited institution.
• Compassionate, attentive to detail, and knowledgeable of procedures and patient safety.
