Licensed Clinical Social Worker (LCSW) - Full-Time

Role & Responsibilities

• Provide discharge planning and psychosocial interventions through assessment, advocacy, referral, and interdisciplinary collaboration for patients.
• Manage policies for assessing and setting goals related to social, psychological, emotional, and financial needs of residents.
• Offer social casework to individuals and families receiving healthcare services, supporting understanding of social and emotional factors affecting health.
• Document services in compliance with state, Medicare, and third-party regulations.
• Assist healthcare providers in understanding patients’ social and emotional needs.
• Prepare social histories, assess patient needs, and facilitate patient and family support.

Qualifications & Skills

• Must hold an LCSW credential and have at least 2 years of relevant experience.
• Strong decision-making, communication, and attention to detail skills are essential.
• Preferably possess a Master’s degree from a CSWE-accredited school.
