This position is a per diem position and can be offered either as a 1099 independent contractor role or a W2 employee role. Pay is $60-$70 per visit depending on being a 1099 contractor or W2 employee.

Job Description

The Licensed Clinical Social Worker (LCSW) will provide individualized counseling, assessment, and therapeutic services to patients in the comfort of their own homes.

Responsibilities

  • Conduct in-depth psychosocial assessments of patients and their families in homecare settings to identify emotional, social, and psychological needs.
  • Provide individual and family counseling services, including crisis intervention and supportive therapy, to help patients cope with illness, aging, or other health-related challenges.
  • Collaborate with a multidisciplinary healthcare team, including physicians, nurses, and therapists, to develop and implement personalized care plans.
  • Educate patients and their families about coping mechanisms, available community resources, and support networks to manage chronic illnesses, disabilities, and emotional challenges.
  • Advocate for patients' needs within the homecare setting, ensuring access to necessary medical, mental health, and community resources.
  • Support and counsel patients with end-of-life concerns, including facilitating advanced care planning and grief support.
  • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory standards and confidentiality laws (HIPAA).
  • Provide ongoing support to patients, including follow-up visits, phone consultations, and coordination with other healthcare providers.
  • Participate in case reviews, interdisciplinary team meetings, and supervision to improve patient outcomes and enhance clinical skills.
  • Maintain knowledge of current therapeutic interventions, social work practices, and healthcare regulations.

Qualifications

  • Education: Master’s degree in Social Work (MSW) from an accredited program.
  • Licensing: Active and unrestricted LCSW license in the state of Kentucky.
  • Experience: Minimum of 3 years of clinical social work experience, preferably in a homecare or healthcare setting.
  • Skills: Strong interpersonal and communication skills, with the ability to build rapport with patients and families; Knowledge of crisis intervention techniques and evidence-based therapeutic interventions; Familiarity with community resources and social service programs; Ability to work independently, manage a caseload, and prioritize tasks effectively; Proficiency in electronic health records (EHR) and documentation standards; Compassionate, empathetic, and committed to patient-centered care.
