Licensed Clinical Social Worker (LCSW)

Role Overview

Responsibilities

• Conduct biopsychosocial assessments and coordinate care plans.
• Handle complex patient care situations, including crisis interventions.
• Navigate Department of Children and Families processes and document according to care plans.
• Collaborate with interdisciplinary teams to ensure continuity of care.
• Manage high-risk populations and utilize community resources effectively.
• Communicate and educate patients and caregivers effectively.
• Support patients across all age groups and adapt to changing needs.
• Travel as needed for patient care and meetings.

Qualifications & Skills

• Valid State License with LCSW or MSW certification.
• Minimum 3 years of experience.
• Experience with charting systems (Epic).
• Knowledge of crisis management, community resources, and healthcare programs.
• Strong organizational, problem-solving, and communication skills.
• Flexibility and adaptability required.
• Must be vaccinated for COVID-19 (including booster) and flu (no exemptions).
