About the role

Imagine empowering individuals within a dedicated patient community to overcome challenges like anxiety, depression, trauma, and addiction. As a trusted clinical partner, you'll deliver compassionate, whole-person, integrated mental health care—guiding patients through stressors related to relationships, family, parenting, and more.

Responsibilities

  • Direct Counseling: Provide compassionate, evidence-based individual and/or family counseling to help patients manage stress, improve relationships, and build healthier coping strategies.
  • Care Coordination: Connect patients to long-term therapy, psychiatric services, or community resources, ensuring continuity of care across settings.
  • Integrated Collaboration: Partner closely with medical providers and care teams to support a holistic view of each patient’s physical and emotional health.
  • Outreach & Education: Drive engagement through wellness initiatives, workshops, and outreach efforts—delivering psychoeducation to patients and client stakeholders.
  • Resource Navigation: Identify and address social determinants of health, connecting patients to community resources, benefits education, and support services.
  • Crisis Support & Advocacy: Assist with crisis navigation and support mental health-related leave (including FMLA documentation) as needed.
  • Team Collaboration: Build strong relationships with clinic staff, leadership, and client partners while adapting to evolving patient and organizational needs.

Requirements

  • Education: Master’s in Social Work (M.S.W.) from a CSWE-accredited program—or a related behavioral health field.
  • Licensure: Active, unrestricted LCSW (or equivalent) in Kentucky (KY), with willingness to obtain additional state licensure (IN, PA, RI, and others as needed).
  • Treatment Experience: At least 5 years of recent experience providing evidence-based individual counseling in a mental health or healthcare setting.
  • Tech Skills: Comfortable navigating electronic medical records, clinical documentation, and basic data entry.
  • Preferred: Experience providing integrated behavioral health services in a primary care setting within the last 3 years.
