Licensed Clinical Coordinator Home Health
About the role
The Home Health Clinical Coordinator is responsible for ensuring a smooth transition from hospital to home by coordinating with facility staff, patients, and families to facilitate timely and appropriate admissions.
Responsibilities
- Meet with patients and families at the bedside to introduce home health services and provide education on care
- Review and validate referral information, including demographics, insurance coverage, and clinical eligibility for home health
- Collaborate closely with hospital case management, discharge planners and physicians to coordinate safe and timely transitions to home
- Serve as the primary liaison between the hospital and home health agency to ensure continuity of care
- Maintain real-time communication with interdisciplinary teams regarding referral status, delays, or changes in discharge plans
Requirements
- Active clinical license in the state of Florida
- Minimum of 2 years of home health experience required
- Strong clinical assessment and critical thinking skills to support patient care planning and coordination
- Support business development efforts within facilities
- Experience with hospital portals preferred
