Responsibilities
- Ensures the continuity of care by working alongside the healthcare providers.
- Reviews home health certifications/recertifications for required documentation.
- Acts as a liaison between the organization and home health companies.
- Acts as a patient advocate.
- Attends clinic mandated and departmental inservices and trainings.
- All other duties as assigned.
Requirements
- Must possess a valid unrestricted Oklahoma license as a Registered Nurse.
- Basic Life Support (BLS) Certification.
- Minimum 5 years experience in Home Healthcare nursing including review of certifications/recertifications.
- Fundamental knowledge of home healthcare rules and regulations.
