About the role

A Licensed Clinical Care Manager contributes to AHF by utilizing their mental health expertise and understanding the Ryan White Program within their specific area. With a caring and knowledgeable focus, you will work collaboratively with the patients, our physician, and the Healthcare Center team to address the needs identified in the patient’s health and psychosocial Comprehensive Needs Assessment and implement an Individualized Care Plan based upon the needs identified, facilitate, and identify service options for addressing a client’s needs.

Responsibilities

  • Provide education and assistance so that eligibility for the program is understood.
  • Understand mental health conditions or substance abuse patterns that interfere with his/her ability to improve the client’s health and social outcomes.
  • Develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for your patients who lack those services.
  • Conduct ongoing retention calls to ensure no gap in retention efforts and to uphold radical cooperation across divisions.
  • Screen clients for complex medical care and psychosocial issues requiring medical case management services.
  • Establish Comprehensive Assessments, Individualized Care Plans, follow-ups, referrals, and direction of clients through the entire spectrum of health and support services.
  • Verify the client’s eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services.
  • Collaborate with providers to mutually develop and establish an action or individual service plan with each client that identifies patient needs, goals, and planned course of action to promote readiness for and adherence to complex HIV/AIDS treatments (i.e., counseling, special programs), and meets identified needs; monitors and documents the changing needs of the client through personal contact and by reviewing the care plan at least every six months.

Qualifications

  • Current and good standing Licensed Clinical Social Worker or Licensed Master Social Worker in the jurisdiction in which services are rendered.
  • A minimum of 1 year of Case Management experience is preferred.
  • Knowledge of Ryan White Services is preferable.
  • Proficient Spanish speaker a plus!
