Position Summary
We are seeking a Lead Youth Family Foundations Housing Case Manager to join CTI’s Youth Services team, located at the Youth Opportunity Center. The Lead Youth Family Foundations Housing Case Manager will provide advanced levels of support to young adults (14-25) and their families entering Youth Family Foundations housing programming. This includes providing advanced level case management, advocacy, referral information and supporting participants in overcoming barriers towards long-term, independent housing.
Essential Functions And Responsibilities
- Train and supervise Youth Family Rehousing Case Management staff
- Receive, assign, and monitor new referrals to Youth Family Housing Programming
- Develop and use network of available services to assist participants and staff
- Support and complete onboarding and program entry documents with staff and participants
- Lead and facilitate team meetings to address program and participant goals
- Collaborate with Youth Family Foundations staff to support participants in housing goals and service planning
- Oversee and support scattered site/residential and rental assistance programming
- Establish and maintain working relationships with housing providers and internal/external partners
- Data input, file management, case management software and all administrative tasks as necessary

Qualifications
- High School diploma or equivalent education
- 2+ years of experience working with youth and young adults
- Ability to work comfortably with pregnant and parenting youth in both group and individual settings, preferably with experience advocating for families
- Minimum of one-year of experience with assessment, advocacy and case management work in a Human Service field
- Demonstrated knowledge of community resources available to low-income individuals and families
- Demonstrated knowledge of supportive housing programming
- Excellent customer service, organizational, oral and written communication skills
- Able to manage multiple priorities and work in a fast-paced environment
- Proficient computer skills in basic computer software and data collection systems
- Excellent crisis management skills
- Adequate attention to detail
- Valid Driver’s License
Preferred Qualifications
- Bachelor’s Degree, preferably in a Human Services related field
- Minimum 5+ years’ direct case management experience may substitute for degree
- Experience with families
- Bilingual skills (English/Spanish, English/Khmer, English/Portuguese)
