About the Role
This position involves providing telehealth triage services, conducting in-person and virtual assessments, and collaborating with medical staff to develop individual care plans. You will also participate in the creation and updating of health and medical protocols, manage staff performance, and ensure compliance with federal and state regulatory guidelines.
Responsibilities
- Meet and maintain all federal and state regulatory guidelines and standards applicable to the position.
- Develop staffing schedules and assignments; provide solutions to resolve staffing gaps.
- Provide telehealth triage services to assess patient needs, evaluate symptoms, answer inquiries, and provide patient teaching.
- Determine urgency and appropriate course of action for patient care.
- Conduct in-person and virtual assessments based on established protocols and accurately document medical findings.
- Gather, collect, and clarify patients’ current condition and past medical history.
- Communicate effectively with the Medical Director regarding concerning findings for patient health and well-being.
- Participate in the development of health and medical-related protocols and procedures for federal programs.
- Collaborate with Lead Medical Coordinators and other medical staff to devise individual plans of care.
- Participate in scheduled meetings, team decisions, and operations.
- Implement Compass Connections safety protocols, including evacuating with children and staff in emergencies.
- Maintain confidentiality of health care records and information in compliance with HIPAA and Compass Connections protocols.
- Adhere to scheduled hours, including availability for on-call, evenings, weekends, and holidays as needed.
- Review, write, and update medical protocols and procedures in collaboration with the Medical Services team.
- Plan, organize, and prioritize daily work using sound judgment.
- Monitor established health care plans for the service population within required timeframes.
- Ensure accurate documentation in accordance with agency policies, regulatory requirements, and program timelines.
- Manage staff performance through feedback, coaching, and corrective action as necessary.
- Meet all program and federal partners’ documentation and process requirements.
- Perform other duties as assigned or required for unforeseen matters.
Requirements
- Registered Nurse license in good standing with the State of Texas Board of Nursing.
- Bachelor of Science in Nursing (BSN).
- At least 5 years of experience as a Registered Nurse, with 2 years of management experience.
- Pediatric nursing experience preferred.
- Current BLS certifications (PALS preferred), First Aid, CPR, and Emergency Behavior Intervention.
- Pass pre-employment and random drug screens throughout employment.
- Provide proof of work eligibility status upon request.
- Pass pre-employment and biennial criminal background checks.
- Demonstrated ability to:
- Work collaboratively with staff, service providers, and professionals.
- Communicate effectively in writing and verbally in English.
- Work in a fast-paced environment while maintaining emotional control and professional composure.
- Maintain computer literacy required for the position.
- Work independently and as part of a multidisciplinary team without intensive supervision.
- Demonstrate knowledge of Compass Connections policies and procedures.
- Provide current physical-medical release and proof of immunizations, titers, or declinations for:
- MMR
- Varicella
- Hepatitis B
- Hepatitis A
- Tetanus
- PPD
- Meningococcal (preferred)
- Ability to perform physical activities such as extensive walking, stretching, and bending.
- Strong professional people skills, including oral, written, listening, and non-verbal communication.

Adhere to scheduled hours, including availability for on-call, evenings, weekends, and holidays as needed or requested.
