About the role
The Mental Health Professional Lead plays a vital role in delivering patient-centered care of the highest quality. They give exceptional psychiatric treatment services to patients and their families, carrying out a range of tasks such as assessments, psycho-education, process groups, treatment planning, discharge planning, and therapeutic counseling help. They use evidence-based interventions in one-on-one, family, and group therapy sessions to achieve the best possible results. Also, they offer leadership and coaching to the care team, acting as an invaluable resource for them.
Responsibilities
- Conducts post-discharge follow-up as indicated
- Collaborates with the multidisciplinary team to assess and coordinate patients' discharge planning needs and readiness for discharge
- Completes assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope
- Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
- Assesses environment and patient's safety status and reports/responds to actual and potential safety issues or changes in the patient's condition
- Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis
- Conducts one-on-one, group, and family psychoeducational and process groups
Qualifications
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
- Masters Degree
- Must have full advanced licensure (LCSW, LICSW, LMFT, LPC, etc)
