Lead Outreach Case Manager
Division: HOPICS | FLSA: Non-Exempt/Full Time (40 hours per week) | Pay Range: $27.81 - $29.00 per hour
About the Role
Under the supervision of the E6 Program Manager and Program Supervisor and as part of a multidisciplinary team, the Lead Outreach Case Manager will perform professional case management services in connection with outreach to, and linkage for, homeless consumers. This role assists consumers and their significant figures in understanding and developing solutions to problems that lead to and result from homelessness. As Team Lead, the incumbent will coordinate team activities, maintain proper documentation, and report team issues to the E6 Program Manager and Program Supervisor. The role includes providing technical guidance, consultation, and supervision to enhance case management techniques for helping homeless clients, while complying with all ethical and legal mandates for documentation and reporting.
Responsibilities
- Conduct immediate needs assessments with adult consumers to complete a comprehensive treatment plan.
- Present within a Multi-Disciplinary Team (MDT) and collaborate with internal and external entities.
- Provide assistance to homeless clients in the field and establish client treatment plans aligned with clinical assessments from therapists, nurses, and substance use counselors.
- Assist consumers in resolving or coping with issues contributing to mental, emotional, medical, and/or substance-related problems.
- Document in accordance with E6 program guidelines, including entering notes, treatment plans, and outcomes into HMIS.
- Provide case management services, including solutions to immediate needs, linkages to benefits, housing supports, mental/physical health programs, substance use programs, food support, hygiene support, and employment or school resources.
- Work with homeless consumers, adults with co-occurring substance abuse issues, and individuals with mental/physical/developmental disabilities.
- Document effectively in accordance with established standards of care on a daily basis.
- Participate in required meetings, trainings, collaborations, and multi-disciplinary team meetings, and present or evaluate client progress.
- Attend additional meetings, training sessions, or seminars as needed.
- Coordinate program activities to meet contractual agreements, laws, regulations, and funding requirements.
- Participate in regular supervision with the Program Manager and/or Program Supervisor.
- Conduct all interventions and treatments in the field.
- Coordinate transportation and transport E6 staff and participants using the Agency van as needed.
- Provide and maintain documentation of homeless status for intake workers and clients, and keep copies in client files.
- Provide HIV/AIDS information, education, and linkages to testing, housing, and services.
- Enter all outreach activity into the HMIS system regularly and ensure accurate entry by all team members.
- Assist with collecting and inputting information for LAHSA-required reports, including QPR and APR.
- Build rapport with community organizations serving homeless individuals, families, low-income households, and other HOPICS target populations.
- Inform, educate, and refer homeless individuals and families about HOPICS programs and services.
- Work evenings, overtime, weekends, or holidays as needed.
- Maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.
- Represent the Agency and Division professionally at meetings and community events.
- Maintain regular attendance and client records/documentation in compliance with HIPAA, CFR42, and funding requirements.
- Ensure all client records/documentation are submitted or arranged for submission upon resignation.
Requirements
- Some credits from an accredited college or university, or at least two years of experience working with homeless individuals.
- Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
- Experience working with homeless clients in the field.
- Effective written and oral communication skills for collaboration with entities.
- Professional appearance, attention to detail, and punctuality.
- Working knowledge of Microsoft Word, Excel, and the Internet.
- Experience working with homeless persons with co-occurring issues (mental health, disabilities, substance use).
- Ability to report legal issues (e.g., Tarasoff, child abuse, vulnerable adult abuse, danger to self/others, grave disability) in accordance with the law.
- Professional experience working within a multidisciplinary team.
- Ability to coordinate submission of client documentation if unable to work.
- If in recovery, a minimum of three years of being drug and alcohol-free is required.
- Verification of employment eligibility, background clearance, and TB test (within 3 months prior to or 7 days after hire, renewed annually).
- CPR and First Aid Certification required within 30 days of employment.
- Valid Driver’s License and auto insurance.
- Knowledge of evidence-based practices and ability to use them in the field.
Working Conditions
This position requires working in diverse environments, including homeless encampments across Service Planning Area 6. The role involves extensive interaction with homeless individuals, individuals with severe physical and mental disabilities, and potential exposure to infectious diseases, stray animals, moving vehicles, and high-crime areas. The incumbent may encounter aggressive or hostile clients and must be prepared to address traumas that may trigger personal trauma. The position requires collaboration with courts, Department of Children and Family Services, LAHSA, law enforcement, and other community agencies. Local automobile travel is required, and the incumbent may be exposed to weather conditions, noise, moving vehicles, odors, fumes, and pollen. The role may also involve working in noisy environments where adults express themselves loudly.
