About Position

Work Location: 88 Vilcom Center Dr., Suite 110, Chapel Hill, NC 27514
Schedule: Monday through Friday, 8:00am to 5:00pm
Travel: As Needed

Job Summary

The Lead Member Care Coordinator (LMCC) provides clinical and administrative oversight, training, and strategic guidance to the member care coordination team, while also delivering direct care and resource coordination to Piedmont patients. Acting as the team lead, the LMCC performs all duties of the Member Care Coordinator role, assists with population health initiatives to improve patient outcomes, and oversees coordination workflows. Additionally, the LMCC tracks performance metrics and provides ongoing team training.

Job Duties/Responsibilities

  • Supervision of member care coordinators
  • Training and development of administrative staff
  • Works in collaboration with management to ensure PHS policy compliance
  • In collaboration with management team insures optimal patient access and efficiency practices are met
  • Synchronizes monthly member care coordinator meetings and creates meeting agenda and minutes
  • Attends payor and network meetings as needed
  • Aid care management teams, internal and external for the coordination of patient care
  • Maintains and reviews member care coordinator productivity and key performance indicators
  • Supports member care coordinators with oversight and guidance as needed
  • Analyzes and synthesizes population health outreach data to optimize and guide the team's ongoing community interventions
  • Aids Population Health Leadership in the execution of population health initiatives as needed

Minimum Qualifications

  • Education: High School diploma or an equivalent combination of education and experience
  • Required: Excellent communication skills
  • Two to three years of care coordination experience in a health care setting
  • Prior management experience of 2 years or more years
  • Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment

Salary Range

$52,806 - $70,107
