Job Summary
The Medical Case Manager (MCM) at Positive Impact Health Centers (PIHC) provides medical case management services to eligible adult clinic patients, adhering to funding guidelines and best practice care models. This role involves working as part of an interdisciplinary team that includes medical providers, behavioral health clinicians, medication access specialists, clinical pharmacists, and supportive services staff.
Essential Functions
- Learn and stay updated on HIV/AIDS-related medical issues and medical case management practices.
- Implement medical case management services to identified clinic patients, following established guidelines.
- Train and supervise assigned staff on relevant standards, procedures, and policies.
- Ensure assigned staff manage day-to-day operations, including coverage of the Resource Line and scheduling patient enrollment visits.
- Develop and implement Individualized Service Plans (ISPs) for assigned patients.
- Assign clinic patients to MCMs for both medical and non-medical services, using Acuity Level scores and service needs.
- Participate in interdisciplinary Case Conference meetings, leading as needed.
- Conduct daily huddles with assigned care teams and document summaries.
- Collaborate with Site Coordinator and Data Team to gather and analyze data for assigned care teams.
- Assist with the orientation of new MCMs at designated PIHC Centers.
- Collaborate with interdisciplinary care teams to address patients' ongoing care needs and make referrals.
- Support Site Coordinator in managing the EMR Medical Case Management pool and assigning clients to MCMs.
- Document patient information accurately and promptly in the agency's electronic medical record system and related reporting software.
- Absorb all state, federal laws and agency policies related to confidentiality and HIPAA.
- Organize and lead weekly team meetings.
- Maintain accurate records of staff time and attendance using the agency's electronic payroll system.
- Conduct performance evaluations with assigned staff.
- Comply with all agency policies and procedures, including the conflict of interest policy.

Qualifications
- Master’s degree in social work, human services, or public health.
- Basic computer skills.
- At least one year of experience in social work or a related service delivery field.
- One year of professional experience in an HIV service setting preferred.
- Administrative or supervisory experience preferred.
- Experience documenting in an Electronic Medical Record (EMR) system required.
- Licensure, if applicable to degree held, is encouraged.
Physical Demands
- Frequent sitting and talking or hearing.
- Occasional walking, reaching with hands and arms, lifting up to 30 pounds.
- Close vision and ability to adjust focus.
