Lead Complex Care Social Worker

Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of LifeBridge Health. Provides professional, comprehensive, family-oriented services to individuals in their choice of residence. Assesses patients' and families' psychosocial, environmental, and financial needs. Formulates, implements, and evaluates a plan of care in collaboration with patient, family, and interdisciplinary team members. Functions as a consultant, educator, and expert practitioner. Serves as an organizational resource to ensure evidence-based practice and to facilitate optimal patient/family care. Is accountable for clinical practice issues within area of expertise.

Responsibilities

• Deliver professional, comprehensive, family-oriented services to individuals in their residence of choice.
• Assess patients' and families' psychosocial, environmental, and financial needs.
• Formulate, implement, and evaluate a plan of care in collaboration with patients, families, and interdisciplinary team members.
• Serve as a consultant, educator, and expert practitioner in social work.
• Act as an organizational resource to ensure evidence-based practice and optimal patient/family care.
• Accountable for clinical practice issues within area of expertise.

Requirements

• Master of Social Work (MSW) required.
• Licensed Masters Social Worker (MSW/LMSW) required.
• 3-5 years of relevant experience.

Pay

$34.18 to $51.27 per hour, based on experience.
