About the Role
The Lead Case Manager will have general oversight and coordination of other case management staff, ensuring they complete intake, coordinated assessments, individualized housing plans, and appropriate community referrals. This role ensures quality service delivery, timely completion of individualized housing plans, and maintains appropriate client contact.
Responsibilities
- Oversee all services and operational activities.
- Provide comprehensive case management, including screening and assessing clients, creating and monitoring individual service plans, crisis intervention, and connecting clients to health, mental health, employment, housing, and other community resources.
- Foster a professional and welcoming environment for clients, guests, vendors, and colleagues.
- Manage and evaluate the performance of assigned staff.
- Develop and maintain professional and supportive relationships with staff and subordinates.
- Assist clients in reaching their case plan goals in tandem with case managers.
- Monitor and evaluate each client’s progression through their case management plan and revise plans as needed.
- Maintain accurate documentation of service objectives, outcomes, and other services in accordance with established guidelines.
- Plan, coordinate, and facilitate life management classes to assist clients in developing functional coping skills; obtain speakers and workshop presenters as needed.
- Meet all federal and state regulatory guidelines and standards applicable to the position.
- Participate in workshops, seminars, and education programs to promote professional growth.
- Compile, prepare, submit, and maintain accurate records, files, forms, statistics, and additional information as required.
- Participate in weekly case staffing with case managers.
- Maintain confidentiality in all areas of the service population and program operations.
- Demonstrate exceptional customer service by prioritizing the needs of clients, families, veterans, or other individuals served.
- Perform other duties as assigned.
Requirements
- Bachelor’s Degree in Social Work, Sociology, Psychology, or a related field (preferred).
- Three years of case management experience, particularly with homeless, low-income, veterans, and their families (strongly preferred).
- Supervisory experience leading other case managers (preferred).
- Demonstrated ability as a servant leader (preferred).
- Must maintain regular and acceptable attendance as determined by the employer.
- Valid Driver’s License with a clear record.
- Daily access to a vehicle without prior notice.
- Willingness to travel to various locations as dictated by business needs.
- Must not pose a direct threat or significant risk to the safety or health of themselves or others.
