Essential Position Functions
- Develop and maintain relationships with key contacts at participating facilities.
- Identify residents who meet plan eligibility requirements.
- Outreach to residents or responsible parties by person-to-person phone calls to present the affiliation information and offer the opportunity to meet with a Provider Partners Benefit Advisor.
- Ensure all activities comply with relevant regulations and industry standards, especially in healthcare-related services.
- Complete and return consent to contact forms (lead cards) to Provider Partners Health Plan.
- Maintains documentation on calls, feedback, and outcomes.
- Makes monthly census checks to identify newly eligible residents.
- Provides education to facility leaders and interdisciplinary teams related to the ISNP plan benefits and eligibility requirements.
- Hosts ISNP Open House events for residents and responsible parties and various facilities.
- Maintains specific knowledge and updates related to the ISNP plan.
- Communicates enrollment status to Senior Vice President of Field Accounting and ASC Leadership.
- Completes other special projects as assigned.
Education And Qualifications
- Previous experience (minimum 4 years preferred) in long-term care or assisted living.
- Proficiency in Excel.
- Previous experience (minimum 4 years preferred) in lead generation, business development, or a related role.
- Ability to travel for on-site facility visits.
