Interventionist (IDD - Mental Health) - Seneca

Responsibilities

• Provide supportive clinical services to 2-3 families at a time, conducting regular to daily meetings with each child/youth and family.
• Deliver crisis intervention, prevention, and resolution strategies.
• Engage in frequent outreach to clients and their families.
• Create personalized service plans tailored to individual family needs.
• Design and implement effective service delivery strategies.
• Collaborate with partner service providers to coordinate comprehensive treatment plans.
• Maintain thorough case management records to ensure effective service delivery.

Requirements

• Education: Bachelors in human services, psychology, social work or other related field required. Masters in Social Work, Psychology, Social-Welfare, Human Services or other related fields preferred.
• Experience: 1-2 years experience in case management, IDD, community based mental health, and other related responsibilities required.
• Driver's License and ability to maintain insurability throughout employment required.
