About the role
The Social Work Intern is responsible for performing professional level casework by assessing individuals for eligibility in the Senior Nutrition program. This includes receiving referrals via phone, email, or voicemail, documenting them in the ServTracker database, conducting home visits or phone interviews, and completing timely reassessments.
Responsibilities
- Receive referrals via phone, email, or voicemail
- Document referrals in the ServTracker database
- Call potential participants and conduct phone interviews or home visits
- Complete timely reassessments to ensure services are not interrupted
- Make monthly contact via phone with meal recipients to check on meal receipt and assess health or living conditions
- Analyze situations and determine appropriate courses of action immediately or within short time frames
- Maintain knowledge of community resources to assist clients with financial, psychological, emergency care, emotional, or other needs
- Stay updated on social work theories, techniques, and practices, as well as relevant social issues
- Communicate with high-risk clients via phone to address inquiries about meal service and available resources
- Document client interactions, services, and referrals according to agency requirements
- Manage case data and documentation using various computer programs
- Comply with state and federal laws, program rules, directives, policies, and regulations
- Collaborate with interdisciplinary teams to provide comprehensive care
- Deliver meals to homebound participants as needed
- Perform other related duties as assigned
Qualifications
- Currently attending a Bachelor of Social Work program with an accredited university
- Four-year degree in a human services field
- Valid Driver’s License with a safe driving history
Supplemental Information
- Physical demands include operating, maneuvering, and controlling equipment, machinery, tools, and materials used in performing essential functions, including computers, stethoscopes, hemostats, portable scales, emergency carts, telephones, etc.
- Must be able to exert up to 50 pounds of force occasionally and/or a negligible amount of force constantly to move objects.
- Direct deposit of paychecks will be made to checking or savings accounts in a bank of your choice.
