About the role
The Community Habilitation Specialist provides assessment, training, and assistance to developmentally disabled individuals enrolled in the HCBS Waiver program and living in a home situation. They work with the consumers in their homes or community, as needed, based on the valued outcomes listed on their Community Habilitation Plan.
Responsibilities
- Facilitate an individual's independence, individualization, productivity, and inclusion in their community.
- Maintain communication with Supervisor(s) about consumers, time off needed, job performance, and relevant issues at least once a month via telephone.
- Communicate with Supervisor(s) and consumer(s)/and family about changes in schedules, lateness, absences, and other issues affecting working with the consumer.
Requirements
- Must be an SCO Family of Services authorized driver and authorized to drive.
Qualifications
- Must have a valid driver’s license.
- Must have experience working with developmentally disabled individuals.
- Must have excellent communication skills.
- Must be able to work independently and as part of a team.
