Responsibilities
- Assist in the day-to-day coordination of the Intensive In-Home program and provide brief behavioral and emotional services to children and their families within a community-based model.
- Serve as a member of a 3-person team with a maximum caseload of 12 families.
- Conduct skill-building activities and provide parenting education.
- Intervene during crises and provide on-call coverage within the intensive in-home team 24/7/365.
- Respond appropriately and timely to crisis situations.
- Develop, review, and follow individual Person-Centered Plans with the Child and Family Teams for each child on the caseload.
- Provide psychoeducation to children and families.
- Facilitate effective step-down to less restrictive services and increased reliance on natural supports over time.
- Collaborate, coordinate, and advocate with collateral agencies and natural supports to meet family and consumer needs in all domains.
- Provide services in the client’s home, school, and community settings under the direction of the Intensive In-Home Therapist.
Requirements
- B.A., B.S., or an Associate Degree in Social Work or a related human service field required.
- Credentials of a Qualified Professional (QP).
- Two years of documented experience with the child/adolescent mental health population.
- Proficiency in Microsoft Office applications.
- Valid Driver's License.
