About the Role
The Intensive Family Coordinator will provide a professional level of case management services as well as direct advocacy services to a caseload of families. The role involves completing family assessments, creating individualized service plans, recruiting foster parents, completing home studies for the referring authority, locating family resources, and providing 24/7 crisis support. Travel is required, along with a willingness to work nights and weekends.
Responsibilities
- Complete family assessments and create individualized service plans
- Provide case management services, including intakes, safety planning, team meetings, and ISP development
- Offer 24/7 support, case contacts, and case record documentation in compliance with state regulations
- Recruit foster parents and complete home studies for the referring authority
- Locate and coordinate family resources
- Provide 24/7 crisis support to the caseload
Requirements
- Minimum High School Diploma or GED Equivalent
- Associates or Bachelor's degree preferred
- Experience working with at-risk youth
- Strong organizational and time management skills
- Proficiency with Microsoft Office
- Experience using an Electronic Health Record (EHR) System is a plus
- Bilingual (Spanish-speaking) is a plus
- Reliable transportation, current driver's license, and current auto insurance coverage
- Flexible hours, including possibility for evenings and weekends
