About the role

This position provides high-quality, client-centered, intensive supportive services to clients in the Ryan White (RW) program. Providing RW case management services, including assessing clients' needs, conducting psychosocial evaluations, developing individual treatment plans, and maintaining documentation of services and staff activities. It also includes providing education and support to client(s), making appropriate referrals and consultations both within and outside the Agency. These case management services will monitor all aspects of the client's care in the client's records to the standard of Miracle of Love (MOL).

Key responsibilities

  • Provide a wide range of client-centered services that link clients to health care, psychosocial, and other services to ensure timely, coordinated access to medically appropriate levels of care and support services
  • Support continuity of care, ongoing assessment of the client and other family members' needs, and personal support systems, that prevent unnecessary hospitalizations or expedite discharge, as medically appropriate, from inpatient facilities
  • Meet services productivity (direct client services) standard of 560 units per month for the provision of Case Management services in accordance with applicable quality criteria
  • Provide intensive case management services including assistance with receiving appropriate mental health/substance abuse treatment, applying for benefits, establishing/maintaining housing, and entering education that will assist with job placement
  • Maintain a tentative caseload of 35 individuals
  • Maintain all required and appropriate documentation for client records and progress notes for assigned cases in accordance with Miracle of Love, Inc. policy and procedures and Orange County Ryan White Program requirements
  • Gain basic information with professional motivational interviewing techniques
  • Maintain weekly contact with client with a minimum of two (2) Face-to-Face engagements per month and/or until the client is virally suppressed, for a period of six months. Following viral suppression, three (3) monthly contacts, with one (1) Face-to-Face engagement
  • Complete initial comprehensive assessment of the client's needs and personal support systems
  • Develop a comprehensive, individualized service plan that is completed with the client
  • Coordinate the services required to implement the plan
  • Monitor clients to assess the efficacy of the plan
  • Coordinate patient prescription distribution with local pharmaceutical companies that are in compliance with the Ryan White Program or complete the referral for ADAP with clients that meet program requirements designed by Orange County Health Department
  • Perform periodic re-evaluation and revision of the service necessary over the life of the client, including client-specific advocacy and/or review of utilization of services
  • Develop and maintain working relationships with other agencies including Corrections/Jail staff and officials, Boards of Mental Health, County Attorney and Public Defenders offices, City Prosecutor's offices, Department of Social Services, Security, law enforcement, and other agencies to assist with providing services for clients
  • Make professional decisions, recommendations, and maintain confidentiality as stated in the MOL Confidentiality Agreement
  • Recognize that individuals with co-occurring disorders are expected, not the exception, and are welcome into treatment
  • Maintain proper appointment scheduling through individual Microsoft Outlook and Electronic System Scheduling calendar
  • Complete preliminary assessment process for persons seeking services, including collecting information concerning client's problem, demographic information, and medical history
  • Initiate admission certification review for all prospective new clients and initiate continued review for all active clients as indicated in accordance with client requirements and agency procedures
  • Complete and document medical forms, psychosocial assessments, and diagnostic summaries in accordance with agency specifications
  • Complete assessments/evaluations to assist in determining appropriate referrals to meet the needs of the client and/or treatment team objectives
  • Develop and update individualized treatment plan/plan of care for assigned cases in accordance with client record and agency procedures
  • Make appropriate referrals and initiate consultations with Miracle of Love and other agencies and organizations in accordance with client's needs, documenting activities in the approved electronic database management system
  • Staff appropriate clients with the program supervisor on an initial and periodic basis
  • Respond to information requests including giving information by phone to providers as well as one-on-one contact; visit agencies, organizations and individuals requesting information as directed by the supervisor
  • Complete on-call assignments and provide back up coverage as requested by supervisor in accordance with agency procedures and schedules
  • Monitor client's participation in planned services; initiate revisions to treatment plan/plan of care, closing/transferring of cases in accordance with Miracle of Love's policy and procedures
  • Complete Miracle of Love's discharge/transfer summaries for assigned cases in accordance with client; audit clinical records upon closing/transfer to ensure full compliance with Miracle of Love's standards
  • Maintain documentation of all staff activity in accordance with Miracle of Love's policy and procedures and client specifications
  • Complete Home Visits to follow-up with clients
  • Implement quality management (QM) standards based on the Clinical Quality Management (CQM) plan; review, revise, and update errors in Provide Enterprise (PE) from the QM task form to ensure QM deliverables are met
  • Attend and participate in staff meetings as required

Qualifications

  • Florida Department of Health 500 and 501 HIV/AIDS certification (MOL will provide this resource during training process if not completed)
  • Bachelor's Degree (Social Work or Public Health Field preferred)
  • Clean criminal history
  • Possess a valid Florida driver's license or have access to reliable transportation

Skills and experience

  • Ability to complete assessments and develop plans for individual cases
  • Ability to follow through on all phases of the assigned areas of treatment plans/plan of care for clients
  • Ability to be an active listener to others and to establish and maintain effective working relationships demonstrating understanding, patience and tact in dealing with clients, personnel and the public

  • Ability to communicate effectively verbally and in writing

  • Ability to maintain complete and up-to-date record documentation
  • Direct Care Case Management experience
  • Bilingual in Creole is a plus
