Responsibilities

  • Lead Care Planning Teams, coordinating various services and supports for MassHealth-enrolled children and youth under the age of 21 who have serious emotional or behavioral health needs.
  • Serve as a liaison between individuals, partnering agencies, and supportive programs or services to facilitate the care of youth.
  • Identify and help families access available services and natural supports for both youth and families, while also monitoring their effectiveness.
  • Provide trauma-informed, wraparound services that prioritize the needs of the youth.
  • Assist parents and caregivers in developing effective advocacy skills.

Requirements

  • Must be 21 years of age.
  • A Bachelor's degree in Social Work, Psychology, or a related field is required; a Master's degree is preferred.
  • Bilingual in Spanish is preferred.
  • Open to maintaining a flexible work schedule, including evenings and occasional weekend hours.
  • Experience navigating child and family-serving systems, as well as collaborating with state agencies, consumer advocacy groups, and/or behavioral health facilities is preferred.
  • Knowledge of Wraparound systems of care and child welfare is preferred.
  • Maintain a valid driver’s license and access to a vehicle as transporting youth is required.

Pay

The pay range for this position is $22.84 to $26.00 per hour. The listed range reflects what we reasonably and in good faith expect to pay for this role at the time of posting the position. The actual compensation an employee is offered may vary based on several factors such as experience, education, licensure, certifications and qualifications.

Schedule

This position requires availability to work from afternoons into the evenings. While weekend hours are available, they are not mandatory. The scheduling for this role is flexible and will be adjusted based on your availability as well as that of the client or family.

Sign-on Bonus

We are offering a $1,000 sign on bonus paid out in full after 6 months of employment.
