About the role
This is a unique opportunity to help build and shape a specialized, interdisciplinary care model designed to meet the complex needs of patients age 16 years and up with moderate to severe IDD. The primary care physician in this practice will play a key leadership role in advancing care delivery, improving outcomes, and ensuring a sustainable, patient-centered model.
Select Responsibilities
- Provide comprehensive primary care services for patients age 16+ with moderate to severe IDD
- Collaborate with interdisciplinary team members to coordinate complex care needs
- Support development and growth of the practice model, including care pathways and best practices
- Build strong relationships with internal and external stakeholders
- Participate in education and advocacy efforts for IDD care across the system and community
- Contribute to program innovation and quality improvement initiatives
Qualifications
- MD or DO, Board Certified or Board Eligible in Family Medicine
- Interest in, or passion for caring for individuals with IDD
- Fellowship training in IDD preferred, but not required
- Open to new graduates/residents with a strong interest in this population
