Responsibilities
- Provides holistic, person-centered nursing care to individuals with Intellectual and Developmental Disabilities (IDD), Autism Spectrum Disorders, and behavioral health conditions.
- Works collaboratively with interdisciplinary teams to promote health, safety, independence, and quality of life, ensuring services align with medical best practices and person-centered values.
- Assists in promoting and maintaining clients’ optimal health through regular review of medical records, coordination of health care services, and participation in discharge planning meetings.
- Develops, updates, and monitors Nursing Care Plans, and participates in client meetings such as ISP and SIS.
- Coordinates and participates in required medical and psychiatric appointments, and communicates health information to medical personnel.
- Guides, supports, and coaches Supervisors and staff in effective communication with medical professionals.
- Collaborates with residential supervisors, physicians, and pharmacists on matters related to medications and their administration.
- Assists in identifying and developing educational resources for clients and families related to health and wellness.
- Supports and educates Direct Support Professionals (DSPs) on clients’ medical diagnoses, prescribed medications, dietary guidelines, mobility limitations, and emergency health protocols.
- Performs Quarterly Nursing Reviews for the Agency’s Senior Centers.
- Alerts and reports identified concerns and risks to the Director of Nursing and relevant Department Director in a timely manner.
- Covers for Behavioral Health nursing staff as needed.
Qualifications
- Registered Nurse with at least 2 years of nursing experience, preferably in Rehabilitation and/or Emergency Nursing, and/or working with individuals with intellectual disabilities and/or Autism.
- Current Pennsylvania RN License.
- Department of Human Services (DHS) Medication Administration & Practicum Observer Certification within the first year of employment.
- Completed and maintained Act 33, 34, and FBI Clearances.
- Valid PA Driver’s license and acceptable driving record.

Pay
Minimum Compensation: USD $34.00/Hr.
Maximum Compensation: USD $34.00/Hr.
