Integrated Services Navigator-Maryvale

About the role

The Integrated Services Navigator provides culturally competent navigation services to patients needing behavioral health or nutritional counseling appointments. This position builds working relationships, provides and connects patients with an integrated services provider, practices excellent customer service, and solves problems while supporting patients throughout the various points from appointment scheduling to completion of visit.

Responsibilities

• Conducts face-to-face screenings to schedule patients eligible for behavioral health or nutritional services.
• Identifies, evaluates, and resolves real and perceived barriers to care, scheduling and completion of integrated services appointments.
• Utilizes evidence-based motivational interviewing tools to encourage patients to schedule and complete integrated services visits.
• Maintains accurate and complete documentation in the patient records.
• Evaluates patient concerns, complaints, and trends to recommend changes to accommodate patient needs and eliminate barriers.
• Maintains regular and predictable attendance.
• Performs other duties as required.

Qualifications

Minimum Qualifications:

• High School Education or GED (General Education Degree) equivalent.
• One (1) year or more experience in Electronic Medical Records.
• Valid AZ driver's license.
• Bilingual in English and Spanish.

Preferred Qualifications:

• Associate degree in related field.
• One (1) year or more of Medical Assistant back-office experience or patient navigator.
