Integrated High Needs Coordinator

Southwest Behavioral & Health Services is seeking a dedicated and empathetic Integrated High Needs Care Coordinator to help fulfill our mission of delivering compassionate care to enhance lives and improve communities. About the role

The Integrated High Needs Coordinator I functions as a member of an interdisciplinary team to provide integrated case management to a caseload of clients with multiple medical and/or co-occurring substance abuse disorders and/or medically ill individuals. The role involves advocating for and supporting clients, engaging with community agencies and healthcare providers to ensure access to necessary services, and reducing emergency room visits and hospitalizations. The coordinator provides clinical support to the team through consultation, education, and information on psychosocial and/or substance abuse conditions, interventions, and resources.

Responsibilities

• Facilitates Child and Family Team (CFT) meetings for assigned families.
• Develops Individual Service Plans (ISPs) using information from comprehensive assessments, the Strengths, Needs, Cultural Discovery (SNCD), and input from all team members.
• Provides information, education, and support to youth, families, and team members on creative treatment planning, care coordination, and development of informal and community supports.
• Ensures the team meets as frequently as necessary in the family’s preferred natural setting to meet their needs.
• Develops, implements, and revises safety/support plans for families as needed.
• Ensures appropriate clinical involvement on teams.
• Provides information to CFT members about community resources and linkages to other services.
• Assists youth and families in securing needed services.
• Maintains accurate, thorough, and current documentation of contact with youth/families, treatment progress, and services received/provided.
• Maintains effective communication with clients, Primary Care Services, mental health providers, family, collateral resources, and other agency staff on behalf of clients.
• Ensures complete and accurate progress notes of all serious incidents and changes in behavior.
• Notifies supervisor of actual or potential challenges with clients, such as non-compliance, decompensation, and crisis.
• Ensures confidentiality of verbal and written youth/family information in accordance with HIPAA standards.
• Applies knowledge of DSM, community resources, psychotropic medications, crisis intervention, and evidence-based practices to work with possibly unstable, treatment-resistant adults, adolescents, and children of various ethnic cultures for co-occurring disorders (GMH/SA/SMI).
• Adheres to the legal, ethical, and professional guidelines adopted by SBH.
• Participates in after-hours, on-call rotation as needed.

Requirements

• Minimum age requirement of 22 years.
• High school diploma or GED and five years of full-time integrated/behavioral health work experience.
• Bachelor’s degree in a related field and a minimum of one year of full-time integrated/behavioral health experience preferred.
• Valid and current Arizona driver’s license and ability to pass motor vehicle screening for insurance purposes to transport clients as needed.
• Fingerprint clearance issued by the Arizona Department of Public Safety or eligibility to obtain upon hire.
