Integrated High Needs Coordinator

About the role

Southwest Behavioral & Health Services is seeking a dedicated and empathetic Integrated High Needs Care Coordinator to join our dynamic Payson team. This role functions as a member of an interdisciplinary team to provide integrated case management to a caseload of clients with multiple medical and/or co-occurring substance abuse disorders and/or medically ill individuals.

The Care Coordinator advocates for and supports clients, engages with community agencies and healthcare providers to ensure access to services needed to increase wellness self-management, and reduce emergency room visits and/or hospitalizations. Additionally, the role provides clinical support to the team through consultation, education, and information around psychosocial and/or substance abuse conditions, interventions, and resources to maintain focus on outcomes and best practices.

Responsibilities

• Facilitates Child and Family Team (CFT) meetings for assigned families.
• Facilitates the development of Individualized Service Plans (ISPs) using information from comprehensive assessments, the Strengths, Needs, Cultural Discovery (SNCD), and input from all team members.
• Provides information, education, and support to youth, families, and team members concerning creative treatment planning, coordination of care, and development of informal and community supports.
• Ensures the team meets as frequently as necessary in the family’s preferred natural setting to meet their needs.
• Develops, implements, and revises a relevant safety/support plan for the family as necessary.
• Ensures appropriate clinical involvement on teams.
• Provides information to CFT members concerning community resources and linkages to other services.
• Assists youth and families in securing needed services.
• Maintains accurate, thorough, and current documentation of contact with youth/families, treatment progress, and services received/provided.
• Maintains effective communication with clients, Primary Care Services, mental health providers, family, collateral resources, and other agency staff on behalf of clients.
• Ensures complete and accurate progress notes of all serious incidents and changes in behavior.
• Notifies supervisor of actual or potential challenges with clients, such as non-compliance, decompensation, and crisis.
• Ensures confidentiality of verbal and written youth/family information in accordance with HIPAA standards.
• Applies good academic and working knowledge of DSM, community resources, psychotropic medications, crisis intervention, and evidence-based practices to possibly unstable, treatment-resistant individuals (adults, adolescents, children) of various ethnic cultures for co-occurring disorders (GMH/SA/SMI).
• Adheres to the legal, ethical, and professional guidelines adopted by SBH.
• Participates in after-hours, on-call rotation as needed.

Requirements

• Minimum age requirement: 22 years.
• High school diploma or GED and five years of full-time integrated/behavioral health work experience.
• Bachelor’s degree in a related field and a minimum of one year of full-time integrated/behavioral health experience preferred.
• Valid and current Arizona driver’s license; must pass motor vehicle screening for insurance purposes to transport clients, based on clinic/program needs.
• Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to obtain upon hire.
