Integrated Care Extender

Role Overview: The Integrated Care Extender supports care coordinators in delivering person-centered services to clients, ensuring they access necessary resources and maintain ongoing connections. Key Responsibilities: Conduct outreach, coordinate services and appointments, provide health education, communicate updates to care teams, manage referrals, participate in team meetings, and document activities accurately within the care management platform (ONEcare). Team Collaboration & Quality Assurance: Work closely with multidisciplinary teams, participate in virtual and in-person training, and support compliance with organizational and regulatory standards through active engagement and precise documentation. Qualifications & Skills: Must be at least 18, possess a high school diploma or equivalent, and have either lived experience with I/DD or TBI, certification as a Peer Support Specialist, or caregiving experience for someone with these conditions. Additionally, candidates need a minimum of two years of relevant paid experience, including one year with the Tailored Care Management population.
