Integrated Care Coordinator
About the role
As an Integrated Care Coordinator at Health Connect America, you’ll serve as a key connector for clients, providers, and community resources—ensuring comprehensive behavioral and physical health support, closing gaps in care, and promoting overall well-being.
Responsibilities
- Conduct assessments and develop individualized, person-centered care plans based on client needs, health goals, and Social Determinants of Health (SDOH)
- Coordinate and monitor services across treatment teams, primary care providers, and community resources to ensure seamless care delivery
- Facilitate follow-up services for recent hospitalizations, life transitions, or other critical events to support smooth transitions of care
- Maintain timely, accurate documentation of care coordination, assessments, and interactions in accordance with regulatory and agency standards
- Support clinic-based operations, including scheduling, reminders, vitals, chart documentation, and direct collaboration with Nurse Practitioners for integrated care clients
- Participate in marketing initiatives, outreach to new referrals, and education of prospective clients to promote engagement and program enrollment
Qualifications
- Bachelor's Degree required — in Human Services with a minimum of two years of experience working with youth or families, or in a non-Human Services field with a minimum of four years of experience working with youth or families
- Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis

As part of your role, you’ll collaborate closely with treatment teams, integrated care staff, and community partners, ensuring that every client experiences comprehensive, coordinated care that addresses both immediate needs and long-term health goals.

Through diligent documentation, adherence to timelines, and proactive monitoring, you’ll help maintain the highest quality of service while promoting positive client outcomes.
