Intake Specialist - Part time weekends
About the role
The Intake Support Specialist plays a critical role in coordinating patient referrals and ensuring a smooth intake process for home health and hospice services. This position serves as a key point of contact for referral sources, patients, and internal teams, ensuring accurate information, timely processing, and excellent customer service.
Responsibilities
- Manage incoming referrals and ensure accurate, timely processing
- Educate callers on agency services and connect them to appropriate resources
- Verify insurance benefits, authorizations, and payer requirements
- Create and maintain patient records in the EMR system
- Identify missing documentation and follow up with referral sources
- Collaborate with care teams, liaisons, and departments to coordinate admissions
- Routinely route referrals appropriately to support timely start of care
- Deliver exceptional customer service to internal and external partners
- Maintain productivity standards and contribute to a team-oriented environment
Requirements
Detail-oriented with strong organizational and critical thinking skills
Effective communicator (written and verbal) with a customer-first mindset
Collaborative team player with a positive, solution-focused attitude
Comfortable managing multiple tasks in a fast-paced environment
High school diploma required; healthcare or medical office experience preferred
Familiarity with EMR systems and insurance verification is a plus
