Job Summary
A professional who assists the Intake Coordinator / Director of Clinical Services / Director of Nursing / Nursing Supervisor with referral intake/data entry/analysis activities, and will gather data for the purposes of accepting referrals for the agency in accordance with regulations and guidelines. Will review medical records to determine eligibility and direct care needs. Services are furnished in accordance with Agency policies.
Responsibilities
- Reviews medical records for initiation of care needs in accordance with physician orders.
- Affords assistance with data collection and entry for the purposes of initiating/continuing of care.
- Affords assistance with insurance eligibility for services.
- Understands and adheres to established Agency policies and procedures.
- Understands and promotes principles of continuous performance improvement.
- Ensures the timeliness and accuracy of all data entered into system software.
- Affords assistance with consultative needs for referral sources, clients and staff.
- Affords assistance in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, state and local laws.
- Participates in studies and research and other administrative functions as assigned.
- Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance.
- Attends staff meetings, and in-services as scheduled.
- Observes confidentiality and safeguards all patient related information.
- Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved.
- Serves as a resource person to referral sources, clients and employees.
- Affords assistance with investigation and reports any problem relating to patient care and/or employee well-being.
- Immediately reports any accident, incident, lost articles or unusual occurrence to the Supervisor.
- Attends pertinent continuing education programs other than routine in-services and shares information with staff.
- Affords assistance in the development of process improvement activities with appropriate data collection, aggregation, analysis, taking action and reporting of results according to Agency’s PI plan.
- Other duties as assigned by the Home Health Intake Coordinator / Director of Clinical Services / Clinical Supervisor.

Works indoors in Agency office

Qualifications
- Minimum of one (1) year of experience in home health, preferred
- Associate’s degree in related field, required
- Minimum of one (1) year experience in healthcare, preferred
- Acceptance of philosophy and goals of this Agency
- Ability to exercise initiative and independent judgment
Physical Demands
Ability to perform the following tasks if necessary:
- Ability to participate in physical activity
- Ability to work for extended period of time while standing and being involved in physical activity
- Moderate lifting
- Ability to do extensive bending, lifting and standing on a regular basis
