Job Summary
The Intake Specialist is responsible for handling initial intake information for patients requesting home care services. Bilingual English-Spanish / English-Creole preferred.
Key Responsibilities
- Receives and processes referrals received via various modalities - Marketers, Call-ins, web leads, community walk-ins, etc.
- Answers phones and gathers all required information to work towards admissions for referrals under MLTCs, MCOs, Private Pay and any other Medicaid centered approvals.
- Inputs data and maintains notes and pertinent information in the referral management system.
- Maintain accurate and organized records of all intake activities and documentation
- Verifies insurances and long-term insurance benefits.
- Basic evaluation of clients' needs to determine eligibility for home care services.
- Communicates with patients and their families throughout the life of a referral through to the start of cares.
- Communicates with contract agencies regarding the status of services and to obtain the authorization for those services.
- Advises clinical team, case acceptance and coordination teams of upcoming enrollments to ensure appropriate staffing.
- Monitors all emails and responds in a timely manner.
- Monitors and follows up on all referral under their workflow in the referral management system on a daily basis.
- Reviews all authorizations received to ensure accuracy for proper entering by the authorization team.
- Communicates clearly, following company guidelines, with other departments to ensure a smooth transition of enrollments for consumers.
- Admits all cases upon completion of Start of Cares (SOCs) in the referral management system.
- Stay updated on LHCSA regulations, Medicaid/Medicare guidelines, and insurance requirement.
- Performs other duties as assigned.

Qualifications
- At least 18 years of age.
- Minimum of High School Diploma or a General Education Development certificate (GED).
- Experience with HHAeXchange a plus.
- Prior experience in Home Health Care a plus.
- Basic understanding of Medicaid, Medicare and long-term care insurance.
- Knowledge of EPACES, BOLT, HHAeXchange preferred.

- Must be able to lift and stoop effectively so as to be able to perform the above-listed responsibilities.
- Must be able to move about inside the office to access file cabinets, office machinery, etc. to be able to perform the above-listed responsibilities.
Pay
$25.00-$26.00 per hour
