Job Responsibilities
- Monitor referrals email and eFax inboxes.
- Assign referrals to Account Managers and/or Intake Navigators for follow-up.
- Review referrals for completeness and contact referral sources for missing information.
- Enter calls, inquiries, and prospective participant information into the central database.
- Upload paperwork into the central database, including forms and documentation.
- Follow up with referral sources, providers, and participants to gather missing paperwork.
- Send weekly reports to referral sources on the status of their referrals.
- Ensure timely responses to all inquiries and referrals.
Qualifications
- High School Diploma required, Associate Degree preferred.
- Minimum 2 years work experience in healthcare, preferably with MLTC, MAP, or PACE programs and/or homecare.
- Knowledge of Medicaid, Medicare, and PACE regulations for eligibility and enrollment.
- Proficiency with CRM platforms; knowledge of Care Compass preferred.
- Proficiency in Microsoft Excel, Word, Outlook, and Teams.
- Excellent interpersonal skills and ability to handle urgent situations.
- Comprehensive understanding of federal and state regulatory requirements with respect to health care eligibility, coverage, and services of various public programs.
- Experience in working independently without significant supervision.
Physical Requirements
- Standing for up to 6 hours a day.
- Sitting for up to 6-8 hours a day for consecutive periods.
- Lifting up to 50 pounds of equipment, baggage, supplies, and other items.
- Bending, squatting, climbing stairs, walking, and maneuvering stairs.
- Agility and fine motor skills for operating and activating equipment, devices, instruments, and tools.
- Sight, visual requirements for reading documentation, papers, orders, signs, and typing/writing.
- Auditory hearing and motor skills for listening and speaking with accuracy.
- Cognitive ability for decision-making, reasoning, and analysis.
